Because the Educational Opportunity Center is federally funded, the office must collect important information from you the first time you come for an appointment. This information will be used to admit you to our program prior to services being delivered. New clients who are under the age of
24 must bring documentation for both themselves
(List A) and their parents/guardians (List B).
List A: All clients must bring each of these items
your current driver's license
your Social Security card
your most recent federal income tax return
documentation of any additional sources of financial assistance (such as food stamps, child support, and any income not included on your federal tax return)
current Social Security benefits statement (if you are receiving benefits)
current disability benefits statement (if you are receiving disability payments)
current LES (leave and earning statement) if you are in the military
List B: Clients under
24 must also bring these items
parents' or guardians' most recent federal income tax return
documentation of any additional sources of financial assistance awarded to parents or guardians (such as food stamps, child support, and any income not included on their federal tax return)
parents' or guardians'
annual statement of Social Security benefits (if they are currently received)
parents' or guardians' statement of disability benefits (if they are currently received)
current LES (leave and earning statement) if either parent or guardian is in the military
